How to get a replacement for lost PRC ID card in 3 easy steps

STEP 1. APPLY ONLINE.

1.1 Log on to online1.prc.gov.ph/Home

You must be registered to use this facility. Sign in if you already have an account.


1.2 Select transaction - "DUPLICATE"

For the purpose of applying for a PRC ID replacement, choose duplicate. Enter profession and license number.

1.3 Set an appointment

Appointment slots are free! No need for fixers. You can easily choose among the available dates in the corresponding PRC Office. Note that you can only reschedule once per transaction.


STEP 2. PAY FOR YOUR ID.

They offer various mode of payments. The best payment option would depend on the PRC office. Try to know whether your chosen PRC office is crowded or not. Paying online will save you more time at a minimal cost.

Fee - PhP 250.00 + convenience fee if paid through selected channels

STEP 3. CLAIM DUPLICATE PRC ID BY TRANSACTING AT THE APPROPRIATE WINDOW IN THE CHOSEN PRC OFFICE ON THE DATE OF YOUR APPOINTMENT.

Most likely, there will be no long queues for PRC ID replacement and you can get your duplicate ID in less than 10 minutes.

Requirements:

Note that online forms can be printed before or after payment.
b. Affidavit of Loss
c. Valid ID

A duplicate PRC ID cards has the word "DUPLICATE" below the photo of the ID bearer.
Same as the original IDs, duplicate IDs also expire on the birthday of the bearer, 3 years from the year of registration or year of last (immediately preceding) renewal whichever is applicable.

NOTE: In case the duplicate PRC ID card will not be available on the date of the appointment and the representative will claim on the provided schedule, representative with proper identification should present special power of attorney/authorization letter from the registered professional and the original claim slip.

  1. Hi. What if the prc got lost less than a year before it will expire? How can you get a duplicate PRC ID?? Is it okay to renew even you lost the ID?
